Notes:
· Before the figure was released, Moore Creations mailed a sample figure out to retailers along with a letter; the figure did not have a number on the right foot and the blister package was stapled to the card
· Inside each case was one Wartorn Kabuki figure and five regular Kabuki figures
· The artwork on the back of the package is from Kabuki: Metamorphosis #3
· The figure was included on pages 8, 10 and 12 of Kabuki: Metamorphosis #9
· Action Figure Digest #71 included images of a prototype figure
